Immediate options to fight this:

Sponsorblock keeps audio/video checksums of known good, or known bad blocks.

Since most Ads are shown many times, keeping a list of known ad signatures would probably be the easiest, then the client just drops any blocks matching a known ad signature.

Youtube agents, like freetube, newpipe, stream a video from two different servers at the same time, and only plays the intersection of them. (this could also be used to find new ad signatures)
