I've never understood what this one is supposed to mean lol
It's a reference to the "Kelly" satirical comics published by The Onion. This is the author persona's self-portrait, which is usually accompanied by a pun relevant to the comic.
I know where it comes from, but I don't get the meaning of it. Like I understand what people mean by  but not Kelly. He looks like he's writing my name down because I'm in trouble.

I think in context like this it just means "this is a pun".
i feel like we post it as the emoji equivalent of a post that would otherwise say "GOOD pun"
Often  is next to a funny caption or * Latest Fad, indicating he's saying something in the comic is a fad for humor.
EDIT: Here's an example
kelly always has this image of him in the bottom right corner doing a pun. the meaning is just "i did a pun"
